Do you bill insurance?
Most of our practitioners are on insurance panels, but vary as to which plans they are in-network with. Please call our front desk, 503.232.5653, and we will be happy to verify your insurance as well as let you know which practitioners are covered under your plan. We also bill for out-of-network coverage.
How does insurance billing work?

Can I download and fill out new patient paperwork before my visit?
Yes! In fact, we encourage it. Please CLICK HERE for all forms.
Can you verify my insurance benefits?
Yes! Call our front desk at 503.232.5653 and we would be happy to verify insurance coverage and get a quote of benefits for you. Please be aware, however, that you should be familiar with your own insurance and a quote of benefits is not a guarantee of coverage.
How soon should I arrive before my first visit?
We suggest arriving 20 minutes before your first visit at our clinic to allow time to fill out new patient paperwork. If you are downloading forms from our site and filling them out ahead of time, please arrive 5-10 minutes early to allow time for processing

What if I can’t make my appointment? What is your cancellation policy
We require 24 hours notice for any appointment cancellations. If you cancel with less than 24 hours notice, or miss your appointment, we charge a cancellation fee. These fees may vary depending on practitioner, and are outlined in our payment policies which are reviewed and signed prior to your first visit.
